Dear Patients:

I am writing to inform you that I will be closing my optometric practice, THE EYE ZONE, David E. Weber, O.D., P.C., located at 11661 Preston Road, Suite 145, Dallas, Texas, 75230, effective May 31, 2026. After 42 years of serving this community, I have made the decision to retire.

I will be available for your health care needs through Friday, May 29, 2026. If you would like to make an appointment, feel free to do so through our website, www.EyeZoneDallas.com or call our office at (214) 368-8825.

Your eye health is very important to me. To ensure continuity of care, I recommend that you arrange to have your future eye care needs managed by another optometrist. You may locate a practitioner through the Texas Optometric Association website (Texas.aoa.org).

Records Request

Your patient records are confidential. A copy can be transferred to you or your new doctor upon your written authorization. The Authorization To Disclose Protected Health Information form is available upon request. Please complete and return it by May 31,2026 to ensure a smooth transition. Upon receipt of the completed form, we will do our best to transfer a copy of your requested records within approximately 15 days.

Personally, it has been a privilege and a pleasure to assist you with your vision care and eye health needs over the years. I wish you the best in your future health and happiness.
